Enteral Nutrition Program

All of our enteral nutrition patients have access to our Registered Dietitian for ongoing monitoring to promote optimal nutrition and improve clinical outcomes. Through continuous monitoring of our home nutrition support patients, we will provide coordinated care between home and hospital and ensure safety and compliance with the prescribed nutrition therapy to prevent complications and readmission to the hospital. Our Enteral Nutrition Program Includes:

• RD will review discharge orders with patient/caregiver and assess understanding of enteral regimen and adaptation to home environment

• Initial nutrition screening for all patients; home visit scheduled for high risk patients, those with additional education needs or when requested by patient, caregiver or healthcare provider

• Dietitian Assessment and Reassessment: All written assessments are sent to patient’s ordering physician
